Passport Days – Preparation Requirements and RSVP

Two dates available!

There are three basic steps to applying for or renewing your passport:

1. Sign up for a time-slot before November 11, 2016
2. Gather/prepare all your documents to BRING to the event (do not mail)
3. Arrive to the Passport Day event on November 15 and November 17, 15 minutes before your selected time-slot

 

1) Sign up for a time-slot before November 11

**Appointment Required: ONE time slot per person only**

2) Gather/prepare all your documents:

 

FIRST TIME Applicants will need… –	Applicants RENEWING a Passport will need… –
1) Passport Application: Download, complete and print: http://www.state.gov/documents/organization/212239.pdf –	1) Passport Application: Download, complete and print: http://www.state.gov/documents/organization/212241.pdf –
2) Citizenship and Identity Evidence: 2 forms of ID Certified Birth Certificate or other as described on the Passport Application ID containing your photograph AND signature such as driver’s license – see Passport Application for other options –	2) Citizenship and Identity Evidence: 1 forms of ID Most recent ORIGINAL passport.(Note, if your name has changed, such as by marriage, you will need to show additional evidence – see application for additional details) –
3) Passport Photos: Photo services will be available at event for an additional fee of $15.00. OR get them ahead of time to bring with you – see photo requirements on application. –	3) Passport Photos: Photo services will be available at event for an additional fee of $15.00. 4) Payment in the form of CHECK or MONEY ORDER onlyCheck #1 – the execution fee of $25.00 made payable to “Postmaster”. If you need a photo taken at the event, ADD the passport photo fee of $15.00 for a total of $40.00. Check#2 – For passport BOOK only – application fee of $110 made payable to “US Department of State”. (For optional services, such as expedited processing or passport cards, see FEES and adjust your payments as required.) –	4) Payment in the form of CHECK or MONEY ORDER onlyCheck #1 – If you need a photo taken at the event, $15.00 made payable to “Postmaster”. Check #2 – For passport BOOK only – application fee of $110 made payable to “US Department of State”.
